Submit this form to CapU Safety in advance of the first planned workday
- This form is initiated by the contractor/employee requesting access to the Children’s Center
- This request will be reviewed and verified by CapU Safety and the Manager of the Children’s Center
- Upon approval, CapU Safety will authorize the request for access and the CapU Liaison will inform the requesting contractor or employee
- All contractors must attend a site orientation prior to starting work and annually thereafter
- Contractors will sign in and out with security each day that they are on campus
- Attach copies of hazard/risk assessments, safe work procedures, training certificates and other safety documents below.

Children’s Center Safety Rules
- All contractors must check-in with security and the Manager of the Childrens Center prior to the start of work
- All contractors will be escorted by a designated CapU employee while working in the Childrens Center
- Work is not permitted in proximity to the children
- Work with anticipated impacts to occupants (noise, fume, debris, etc.) may not be done during normal operating hours.
- Contractors are not to park in the Children’s Center parking spots– these are for parents
- Smoking is only permitted in designated smoking areas
- No foul language
- A Criminal Record Check may be required, depending on the type and duration of the work. Where possible this will be identified prior to the award of the contract and part of the contract requirements.
