Submit this form to CapU Safety in advance of the first planned workday
- This form is initiated by the person requesting to complete work (including inspections) in a confined space
- This request will be reviewed and verified by CapU Safety prior to approval
- Upon approval, CapU Safety will authorize the request and the CapU Liaison will inform the requesting party
- All contractors must attend a site orientation prior to starting work and annually thereafter
- Contractors will sign in and out with security each day that they are on campus
- Attach copies of hazard/risk assessments, safe work procedures, training certificates and other safety documents below

The contractor requesting confined space access must provide CapU with copies of their confined space program including specific assessments and plans, and a rescue plan where required, for review prior to the work taking place. Attach documents below
It is expected that the contractors confined space program, procedures, and plans meet the requirements of the Occupational Health and Safety regulations and guidelines Part 9.
Confined spaces permits and rescue plans will be reviewed and verified at the work location by a member of the CapU health and safety team before work begins each day.
